• Health insurance information
• Contact info for two emergency contacts

Please note:

• The application must be filled out completely before it can be submitted. Partially completed forms will not be saved. Foundation for Peace reserves the right to refuse any trip applicant.
• When you submit the application, you will receive an email confirmation. Expect to hear from the trip leader regarding trip preparation.
